RUBISLAW PLACE MEDICAL GROUP CHRONIC DISEASE MANAGEMENT NURSE 0800-1630 Monday-Friday Salary dependent on experience We are looking to recruit an experienced Chronic Disease Management Nurse who will work independently as part of our friendly team helping us offer the best possible care to our patients suffering from asthma, COPD, diabetes and chronic heart disease.
