Overview
I am looking for an RGN to be apart of the Promoting Independence team which is based in Worcester. The main duties include the telephone triage of referrals from local and out of area, acute and community hospitals, working closely with hospitals and community teams to help provide a seamless journey back to their own home, wherever possible. On the basis of this trusted assessment, short term support is requested from either Promoting Independence or the Neighbourhood Teams so that individuals can go home to complete their recovery as appropriate. You will be working as part of a team consisting of other nursing/therapy staff, administrators and team managers.
