Overview
Age UK Lewisham and Southwark is launching a brand new Social Prescribing service centred around the Emergency Department of University Hospital Lewisham. We&39;re going to support adults (18) accessing the Emergency Department with their non-medical needs, thereby improving their health and wellbeing while at the same time reducing the pressure on hospital staff. We are looking for a new Link Worker to help people find and use community support, services and activities. This will be through face-to-face appointments in the hospital, home visits, telephone calls and letters. As well as this, you&39;ll be point of contact for the new project across the hospital. We need confident self-starters, able to work in busy enviornments while building good relationships with a variety of professionals. This will be rewarding role giving you the opportunity to make a real difference. Not only will you directly help the people we support to improve their lives, indirectly you will be doing vital work to ease the pressure on University Hospital Lewisham and free up resources for everyone in the borough. This is a pilot project, so you will also provide valuable input and shape how the service itself is delivered. Further, there will be the opportunity to attend events, forums and meetings to broaden your understanding of Health and Social Care and build experience working in the voluntary sector. You will also be working closely with our other social prescribing projects in the borough, Community Connections and SAIL, strengthening our offer of support in Lewisham.
